Work Schedule: Part-Time approximately 25 hours per week. This is a contracted position.

Position Summary

Provides Early Intervention nutrition and feeding services to children birth to three and their families within the context of established Early Intervention practices and in accordance with each childs Individualized Family Service Plan (IFSP). Promotes the achievement of children and family outcomes related to nutrition feeding and healthy growth and development. Works collaboratively with families service coordinators and other IFSP team members to provide education guidance and support. Reports to the Early Intervention Manager.


Key Responsibilities

  • Supports the achievement of children and family outcomes related to nutrition and feeding.

  • Conducts nutritional assessments and collaborates with families to develop individualized nutrition and feeding goals that are age and developmentally appropriate.

  • Provides direct services to families in their homes or community settings consistent with Early Intervention best practices.

  • Clearly communicates and collaborates with all members of the childs IFSP team to ensure coordinated service delivery.

  • Ensures completion of all required documentation within established Early Intervention timeframes and standards.

  • Maintains compliance with all Early Intervention federal state and county regulations policies and procedures as well as HIPAA requirements.

  • Maintains active Pennsylvania licensure as an LDN and completes all continuing education requirements for licensure State and County Early Intervention programs.

  • Attends required State and County Early Intervention trainings as requested.

  • Upholds Achievas mission values and commitment to providing inclusive family-centered supports.

  • Performs other duties as assigned.

Qualifications

  • Licensed Dietitian Nutritionist (LDN) in Pennsylvania.

  • Minimum of 12 years of pediatric experience preferred.

  • Experience providing Early Intervention or family-centered care is strongly preferred.

  • Must possess valid clearances and meet all State and County Early Intervention requirements.

Additional Information:

Independent contractors have the flexibility to set their own schedules and caseloads. Services are provided primarily in family homes and community settings.
